REQUEST FOR PROPOSALS
Permanent Supportive Housing Foundations
(PSH Foundation RFP)

INVITATION

Southeast Health Group (SHG) is pleased to announce a request for proposals (RFP) Permanent Supportive Housing (PSH) for foundation’s installation to a building located at 3500 First St S, Lamar, CO 81052. You are invited to submit a proposal for the services outlined in the Scope of Work.
All Proposals are due no later than 4:00 pm local time on August 31st, 2022.
The selection process shall not be based solely upon project cost information. Due to the mission-critical nature of the services to be performed, experience, references, qualifications, good customer service, and reliability will be important factors in selecting an organization. Due to the time-sensitive nature of the services to be performed, the successful bidder will be expected to have a labor force large enough to complete the project in the amount of time provided by the Owner.

PURPOSE OF THIS RFP

Southeast Health Group is a not for profit company located primarily in the lower Arkansas Valley of Southeastern Colorado providing behavioral healthcare, primary care, vocational training, and transitional housing to residents throughout the area. SHG is seeking a qualified contractor to install six stem wall duplex foundations with conditioned crawl spaces for our PSH Project.

SCOPE OF WORK

The scope of work shall consist of install and pouring 6 duplexed stem wall foundations on the West and South sides of the lot in accordance with the site plan, excavation company and utility layout.

PLANS, ACCESS, AND SPECIFICATIONS

There will be an optional site visit to view the site on August 17th at 9am local time. For plans, see Exhibit A: Site Plan, Foundation Detail, Utility Layout and Scope of Work attached to this RFP document.

PROJECT COMPLETION DATE

Time is of the essence due to project must be completed prior to October 31st 2022.

 SELECTION PROCESS

A selection committee will review and evaluate the Proposals submitted. SHG representatives may request additional information. After review of the Proposals, the company awarded the contract will be contacted the following Thursday September 1st 2022.

SUBMITTAL REQUIREMENTS/PROPOSAL CONTENTS

All interested firms are required to submit an electronic file in .pdf or Microsoft Word format of the Proposal sent via email to perform the requested services. The Proposal must include demonstrated experience. If substitutions to the scope or specifications are included as part of the contractor’s bid please clearly and specifically note those on The Proposal.

MISCELLANEOUS

• Proposals received prior to the deadline specified above may be withdrawn or modified by written request of the proposer. To be considered, however, the modified Proposal must be received by the time and date specified above.
• This RFP does not commit SHG to award a contract, to pay any costs incurred in the preparation of a Proposal for this request, or to procure or contract for services.
• SHG reserves the right to reject any or all Proposals, call for new Proposals, waive any informalities in Proposals or to award a contract to the next most qualified organization if the selected organization does not execute a contract within 15 days after notification of the award.
• The final contract for services shall be negotiated by SHG. Acceptance of a form contract as part of the Proposal in no way binds SHG to those terms and conditions.
• Prospective organizations shall disclose any financial, business, or other relationship with SHG, its board of directors and/or employees.
• The selected organization shall comply with all applicable federal, state, and local laws, ordinances, rules, and regulations in regard to nondiscrimination in employment because of race, creed, color, ancestry, national origin, religion, sex, marital status, age, medical condition, pregnancy, disability, or other prohibited basis.
• Interested firms should not solicit employees or board members of SHG regarding this RFP. Doing so shall be grounds for dismissal from consideration.
