C.N.A 32 – 40 hours/week | $12.32 – $16.07
• If you don’t have certification, we can work with you starting as Support Staff and working up to a CNA within the first year
• LTCC – Provides direct personal care to residents and maintain a safe, clean environment.
• Hosp – Assist the Charge Nurse for patient care plan implementation by contributing information in day-to-day management of patient care.
Requirements: Prefer CPR Certification and CNA License in the state of CO


Licensed Practical Nurse 36-40 hours/week $19.67 – $24.95
• To provide direct patient care, meeting both psychological and physical needs of the patients in accordance with physicians’ orders OSHA, nursing and department guidelines.
• The LPN fulfills these duties to contribute to the continuity and quality of care that fosters the best interest and well-being of all patients and their families.
Requirements: Licensed Practical Nurse in Colorado and Basic Life Support (BLS) Certificate.


Registered Nurses 32–40 hours/week $28.00 – $35.00
• Hosp – Reporting and accountable to the CNO. Responsible for coordinating and delivering patient care utilizing the nursing process.
• LTCC – Responsible to provide direct and indirect total nursing care of residents. Responsible for the direction and supervision of CNAs performing resident care.
• Home Health – To provide direct patient care, meeting both psychological and physical needs to the patients in accordance with physician’s orders, OSHAs nursing and department guidelines.
Requirements: Current RN license in the state of CO.


Physical Therapist 40 hours/week $45.63 – $54.68
• Evaluates patient upon initial encounter to assess impairment and current condition to estimate rehabilitation potential.
• Plans physical therapy treatment program for each patient, based on the assessment, including training in functional mobility, use of assistive devices, therapeutic exercise, balance and coordination retraining, massage, heat, and cryotherapy.
• Applies diagnostic and progressive muscle, nerve, joint and functional ability tests.
• Directs and aids patient in therapeutic regimen.
• Delegates routine care to supportive personnel as appropriate.
• Acts in supervisory role to ensure proper execution of tasks.
• Provides the necessary billing documentation for billing purposes.
Requirements: Current Colorado Physical Therapy License. Current certification in cardiopulmonary resuscitation is required. Must have an available working automobile, valid driver’s license and proof of insurance.


Physical Therapy Assistant 40 hours/week $22.63 – $28.28
• Performs delegated treatment procedures such as therapeutic exercises to maintain and restore strength, endurance, coordination and range of motion
• Encourages and motivates patients to participate in the treatment program by discussing and explaining their treatment goals and objectives.
• Writes progress notes in patients’ physical therapy records noting reaction and response to the therapy treatment.
• Maintains accurate records of therapy treatments performed and the duration and frequency of treatments, which are reviewed and countersigned by the Registered Physical Therapist
Requirements: Current Colorado Physical Therapy Assistant License. Current certification in cardiopulmonary resuscitation is required.


Occupational Therapist 40 hours/week $38.70-$48.38
• Counsels patient and family according to degree of disability or impairment.
• Completes all required documentation thoroughly and in a timely manner.
• Participates in educational programs and in-service meetings.
• Responsible for managing patient caseloads in all clinical settings from screenings and evaluations through discharges.
• Compliant with State and Federal Regulations.
• Active participant in Interdisciplinary Team approach to patient care.
• Provides the necessary billing documentation for billing purposes.
Requirements: Licensed Occupational Therapist in Colorado and current CPR certification. Must have an available working automobile, valid driver’s license and proof of insurance.


Certified Occupational Therapy Assistant (COTA) $22.63 – $28.28
• Provides occupational therapy treatments to patients according to treatment plans developed by the Occupational Therapist.
• Assist the Occupational Therapist when performing tests, evaluations, and complex treatment procedures. Researching, understanding, and complying with Federal and State regulations.
• Observe, record and report to the therapist the conditions, reactions and responses related to assigned duties.
Requirements: Current license as Colorado Occupational Therapy Assistant and current CPR certification.


Radiology Technologist 32-40 hours/week $21.55-$26.94
• The primary function of the RT is to set up and operate the equipment to produce standard and special radiographs of portions of the body for purposes of diagnosis and treatment.
• To be accessible to patients and other personnel when testing is being performed.
• Perform X-ray and CAT scan examinations in accordance with prescribed radiation safety procedures.
• Schedule patients for x-ray examinations; set up and explain x-ray procedures;.
Requirements: Associate degree geared towards Radiology with current certification by American Registry of Radiologic Technologists (ARRT). Education in Computed Tomography (CT) and training required within first of hire date. Experience in CAT scan is preferred.


Ward Clerk 32-40 hours/week $13.23-$16.54
• Relays resident needs to nursing staff; establishes a positive image with families, community members, and facility departments;
• organizes all paperwork and ensures adequate and complete documentation of resident care; assists in developing or revising various forms and entering new monthly data on nursing forms, utilizing the computer; carries out special assignments as requested by supervisor.
• Attend educational and other mandatory meetings as required. Regular attendance and punctuality.
Requirements: High school diploma or equivalent.


Food Service Worker II 32-40 hours/week $12.92-$15.75
• Performs a variety of cleaning-related and other general duties.
• Performs assigned inventory and ordering. Assists in receiving and storing food and supply deliveries.
• Assists in light cooking, such as pancakes, French toast, eggs, etc.
• Assists with special functions as directed.
• Act in compliance with federal and state sanitary regulations as well as facility infection control guidelines and dietary policies and procedures.
Requirements: Preferred High School Diploma or equivalent.


Life Engagement Aide 32-40 hours/week $12.56-$15.99
• Responsible for assisting in the ongoing assessment of residents
• Planning and conducting regularly scheduled activities and special programs and services related to the needs of the residents.
Requirements: Must have CNA or become a CNA – ASAP; Must have valid Driver License


Housekeeping Aide 20-32 hours/week $12.32-$15.32
• Maintain floor surfaces in clean and aseptic condition; dust and wet mop tile floor surfaces.
• Maintain a dust free environment by high dusting flat surfaces
• Maintain clean environment in patient rooms, sweep and mop tile floors and/or vacuum carpet.
• Clean patient rest rooms, vanity area, shower and commode. Remove trash from patient rooms,
Requirements: Preferred High School Diploma or equivalent.


Maintenance Tech III 32-40 hours/ week $16.08 – $20.10
• Performs day to day operations of the HVACR systems.
• Maintains refrigeration records per State and Federal requirements.
• Performs a wide variety of general building maintenance repairs and services.
• Responsible for the completion of all maintenance service requests as assigned. Work within expense limits established.
Requirements: Preferred High School Diploma or equivalent; Certified HVAC, Electrical or Plumbing.
